Job summary

Qualifications

• Current, unencumbered CNA certification in the state of practice.
• Successful completion of screening/competency test with ≥80%.
• TB results and CPR current.
• Flexible, organized with sound judgement.

Responsibilities

• Document tasks and report to RN Case Manager.
• Assist patients with activities of daily living.
• Monitor and document vitals.
• Transport and assist with mobility and grooming.
• Follow Plan of Care/Home Health Aide Assignment.
• Collaborate with health care team.
