Responsibilities

• Assist residents with activities of daily living (bathing, dressing, grooming, toileting).
• Support transfers, mobility, and positioning per care plans.
• Assist with meals, including setup, feeding, and monitoring intake.
• Obtain and document vital signs and other resident information.
• Observe changes in condition or behavior and communicate to the nurse.
