Job summary

Monarch Communities Spring Meadows in Trumbull, CT is seeking compassionate Care Managers (CNA) to assist residents with daily living according to their care plan. This per diem, part-time role covers day, evening, and overnight shifts, with schedules including 6:00am-2:00pm, 2:00pm-10:00pm, or 10:00pm-6:00am.

You will provide personal care, escort residents to meals and activities, document services, and report any changes in condition to your supervisor.

Qualifications

• Experience with elderly population; dementia experience preferred.
• Care experience as CNA is desirable.

Responsibilities

• Provide resident care including bathing, dressing, toileting, and feeding as per care plan.
• Maintain safety, comfort, dignity and confidentiality of residents.
• Escort residents to meals and activities.
• Document services provided during shift.
• Report changes in resident condition to supervisor immediately.

Job Description

Led by the community's Director of Health and Wellness, we are looking for compassionate and dedicated Care Managers (CNA) to assist residents in all aspects of their daily life as indicated in the resident's care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities.

Schedule: Per Diem: Day, Evening, and Overnight shifts

Shifts:6:00am - 2:00pm, 2:00pm - 10:00pm, or 10:00pm - 6:00am

Responsibilities and Duties

• Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan
• Maintain the safety, comfort, dignity, and confidentiality of all residents
• Escort residents to meals and activities
• Ensures all assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts
• Document all services provided to your assigned residents during your shift
• Report any change of condition in a resident immediately to your supervisor

Qualifications

• Knowledge and experience in working with the elderly population, dementia experience preferred.

Physical Abilities

• While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
• An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
