Job summary

Adoration Health is seeking a compassionate Hospice CNA to join our Altoona, PA team. This full-time role supports patients in a 1:1 care setting, with a schedule Monday–Friday 8am–5pm, serving Bedford County, PA.

You will provide dignity and comfort, assist with personal care and mobility, collaborate with the hospice team, and maintain documentation; benefits include medical, dental, vision, HSA/FSA, tuition discounts, 401(k), PTO, and mileage reimbursements.

Qualifications

• Minimum of six months experience including providing personal care.
• Must be 18+ years of age.
• CPR certification required.
• Ability to read and follow written instructions.
• Comfort with hospice philosophy and caring for terminally ill patients.

Responsibilities

• Provide 1:1 care to support bathing, dressing, mobility and repositioning.
• Monitor and report changes in patient's condition.
• Assist with ambulation, transfers, ROM exercises, and safe equipment use.
• Medication assistance per state regulations.
• Offer companionship and emotional support to patients and families.
• Light housekeeping; assist with meals and maintain a clean space.
• Maintain accurate and timely documentation.
• Collaborate with hospice interdisciplinary team.

Responsibilities

• Personal Care: Help with bathing, dressing, toileting, mobility, and repositioning
• Monitor & Report: Observe and report changes in the patient's condition
• Mobility: Assist with ambulation, transfers, ROM exercises and safe equipment use
• Medication Assistance: Help with self-administered medications per state regulations
• Emotional Support: Offer companionship and comfort to patients and families.
• Light Housekeeping: Change linens, assist with meals, and maintain a clean space.
• Documentation: Maintain accurate and timely documentation
• Team Participation: Collaborate with hospice interdisciplinary team

Qualifications

• Meet the training requirements of the state of practice
• Minimum of six months experience including providing personal care
• Must be 18+ years of age
• Complete a minimum of 75 hours of classroom and supervised practical training, 16 of the 75 hours of classroom must be complete prior to a mandatory 16 hours of practical training
• Possess and maintain current CPR certification
• Ability to read and follow written instructions and document care given
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill
• Ability to work with little direct supervision
• Ability to handle death/dyingStrong oral and written communication skills
• Good Organizational Skills
