Responsibilities

• Safely transporting residents to and from appointments, therapy sessions, and other activities.
• Providing one-on-one assistance with daily living activities, such as bathing, dressing, and grooming.
• Maintaining accurate records of resident transportation and care activities.
• Collaborating with the care team to develop and implement individualized care plans.
• Demonstrating a strong understanding of resident needs and preferences, and adapting care accordingly.
• Conducts resident transportation, ensuring safe and efficient transport to and from appointments, therapy sessions, and other activities.
