Responsibilities

• Escort residents to appointments and activities, assisting mobility and wheelchair transfers.
• Provide wheelchair assistance and ambulation support to ensure safety.
• Monitor changes in resident condition and report to nursing staff.
• Assist with bathing, dressing, and grooming.
• Collaborate with the care team to implement individualized care plans.
• Maintain accurate records of resident interactions and care provided.
