Job summary

Senior Helpers of North West Hartford, CT, is seeking a compassionate Certified Nursing Assistant to join as Case Coordinator. This dual-role blends office administration with in-home care coordination, connecting clients with caregivers and coordinating start of services.

The ideal candidate will have CNA credentials, strong organization and communication skills, and experience with HIPAA compliance and paperwork management to support safe, dignified home care.

Qualifications

• Valid CNA credential recognized by the State of Connecticut, with no restrictions or disciplinary actions.
• Valid driver’s license, reliable transportation and valid auto insurance.
• Previous experience in a care-giving, healthcare, home-care, senior-care or related client-facing environment.
• Strong administrative and organizational skills, including accurate record keeping and scheduling.
• Excellent verbal and written communication skills.
• Ability to conduct client visits and assessments in accordance with training, agency procedures and the limits of the role.
• Commitment to confidentiality, safeguarding, professional conduct and person-centered care.

Responsibilities

• Conduct initial in-home assessments and reassessments, following Senior Helpers policies and approved assessment tools.
• Coordinate caregiver assignments according to client needs, caregiver availability, skills, experience, location and continuity-of-care requirements.
• Maintain accurate client records, care documentation, caregiver files, schedules and service commencement information within the agency’s approved systems.
• Build trusting relationships with clients, families and caregivers through clear, compassionate and timely communication.
• Escalate changes in condition, safeguarding concerns, unmet needs and other risks to the appropriate manager or healthcare professional.
