Job summary

Senior Helpers North West Hartford is seeking a compassionate and organized Case Coordinator (CNA) to join our Client Services and Operations team in Connecticut. This full-time role combines office administration with in-home client assessments, coordinating caregiver assignments and ensuring safe, dignified, person-centered care at home.

The ideal candidate will have a valid CT CNA credential, strong communication skills, and experience in home-care settings.

Qualifications

• Valid CNA credential recognized by the State of Connecticut.
• Valid driver’s license, reliable transportation and auto insurance.
• Experience in a care-giving or home-care environment.
• Strong administrative and organizational skills including record keeping and scheduling.
• Excellent verbal and written communication skills.
• Ability to conduct client visits and assessments per training.
• Attention to detail and ability to manage priorities.
• Basic computer literacy and willingness to learn scheduling/ care-management systems.
• Commitment to confidentiality and professional conduct.

Responsibilities

• New client assessments and care coordination.
• Coordinate caregiver assignments based on client needs and availability.
• Maintain client records, care documentation and service start information.
• Engage clients and families to understand expectations and agree on support.
• Coordinate service starts and caregiver on-boarding.
• Maintain confidentiality and HIPAA compliance.
• Support daily office operations and scheduling.
