Job summary

Live Well Home Care is seeking a compassionate Certified Nursing Assistant to provide in-home care to clients in their own homes and communities. You will assist with daily living activities, personal care, and essential household tasks while following a detailed care plan.

You will deliver companionship, monitor client safety, and coordinate with the Branch Manager to ensure high-quality care. This role emphasizes dignity, respect, and reliable support for seniors and differently-abled

Qualifications

Must have valid CNA credentials.
Provide in-home care including ADLs and personal care.
Adhere to care plans and direction from Branch Manager.

Responsibilities

Assist with activities of daily living: ambulation, bathing, dressing, toileting.
Provide personal care and maintain client safety in the home.
Perform housekeeping tasks: light cleaning, laundry, meal prep.
Offer companionship and emotional support.
Remind and assist with medications according to care plan.
Escort clients to appointments and errands as specified.
Support meal planning, grocery shopping, and meal prep.
Document care provided and report changes to supervisor.
Follow written care plans and agency policies.
Monitor supplies and notify supervisor of needs.
Maintain professional and respectful interactions with clients.
