Job summary

Live Well Home Care seeks compassionate in-home care aides to support clients in their own homes. You will assist with activities of daily living, provide meal preparation and light housekeeping, and ensure a safe, respectful environment aligned with each client’s care plan.

Responsibilities include companionship, transportation support, and coordinating with the care team to deliver consistent, high-quality care tailored to individual needs.

Responsibilities

• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming
• Provide meal preparation and light housekeeping to support client independence
• Offer companionship, social interaction, and mental stimulation
• Escort clients to appointments and assist with transportation when needed
• Monitor safety and report changes to the Branch Manager
• Follow care plans and document care provided
• Assist with medication reminders as directed
• Support family caregivers and respite services as outlined in the plan
