Focused Post Acute Care Partners is seeking a Certified Nurse Aide to join our Linden team in Texas. The role focuses on assisting residents with activities of daily living, ensuring safety and dignity, and following the charge nurse’s schedule.

You will use PointClickCare for documentation while contributing to a caring, compliant environment. The position emphasizes strong English communication, physical ability to assist transfers, and a commitment to quality resident care in a long-term care

Qualifications

• High school diploma or equivalent (GED).
• Must have current Nurse Aide certification in the state of Texas.
• Must be in good standing on the Nurse Aide Registry in Texas and Misconduct Registry.
• Ability to understand, remember and carry out verbal and written instructions in English.
• Ability to speak clearly and hear well enough to communicate with residents.

Responsibilities

• Ensure resident safety.
• Bathe, shower, shampoo, shave, comb hair, dress residents.
• Follow resident assignment schedule by the charge nurse.
• Lift, move, and transfer residents as required.
• Respond to call lights promptly and assist or feed residents.
• Keep residents clean and dry, provide incontinent care.
• Provide supportive, protective, and safe environments for residents.
• Other duties as assigned.
• Utilize EMR charting; PointClickCare.

POSITION SUMMARY:

Responsible for assisting residents with activities of daily living to promote resident independence and dignity.

QUALIFICATIONS:

• High school diploma or equivalent (GED)
• Must have current Nurse Aide certification in the state of Texas
• Must be in good standing on the Nurse Aide Registry in the state of Texas and Misconduct Registry
• Ability to understand, remember and carry out verbal and written instructions in English
• Ability to speak clearly and hear well enough to communicate with residents

ESSENTIAL FUNCTIONS:

• To ensure resident safety.
• Bathe, shower, shampoo, shave, comb hair, dress appropriately, nail care of any residents assigned
• Follow resident assignment schedule as made out by the charge nurse
• Lift, move, and transfer residents as required
• Answer call lights in a timely manner
• Assist or feed residents
• Keep resident clean and dry, toileting or providing incontinent care
• Provide supportive, protective, and safe environments for residents
• Other duties as assigned
• Utilize electronic medical records charting; point click care

PHYSICAL REQUIREMENTS:

Have the ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching and lifting up to 50 pounds with or without reasonable accommodations
