Job summary

Colorado PACE in Denver Metro is seeking a full-time Certified Nursing Assistant for home care. Schedule is Monday–Friday, 8am–5pm with weekend availability as needed; flexibility preferred. Pay range $20.70–$24.50 hourly, with mileage/overtime pay and generous benefits, including HSA, PTO, holidays and 403(b) match.

You will assist patients with personal care, light housekeeping, meal prep, and companionship per Plan of Care while documenting care and collaborating with the primary nurse.

Qualifications

• High School Diploma or equivalent.
• One year of professional experience with frail/elderly population, home care or geriatric care.
• Valid Colorado Driver’s License and proof of Auto Insurance.

Responsibilities

• Assists patients with personal care activities according to the Plan of Care, including hygiene, mobility, and elimination.
• Assists patients and families with home maintenance activities per Plan of Care including light housekeeping and meal preparation.
• Performs support activities as indicated on the Home Health Aide plan of care, including providing companionship and respite for families.
• Establishes therapeutic relationships with patients and families from diverse backgrounds.
• Provides emotional support to patients and families to foster independence.
• Reports to primary nurse regularly and when changes in patient condition warrant.
• Uses universal precautions and appropriate PPE as needed.
• Maintains competencies to perform patient care tasks (bed bath, transfers, oral care, Foley, etc.).
• Cleans durable medical equipment as necessary.
• Documents care provided and patient/family response per policy.
• Participates in department and agency quality improvement activities.

Pay Range: $20.70 - $24.50 / hourly

Supplemental Pay: Based on position, schedule and/or availability: Paid Mileage/Overtime

Mileage and Expense Reimbursement: Sixty-two and a half cents per mile – one of the highest in the industry!

Rewarding Work You Will Do

• Maintains competencies to perform patient care tasks such as bed bath, shower, change of bed linens, patient transfers, oral care, Foley catheter, etc.
• Cleans durable medical equipment as necessary.
• Documents care provided and the response of the patient/family per agency policy.
• Participates in departmental and/or agency-wide quality improvement activities.

What We Will Love About You

• Minimum Education: High School Diploma or equivalent.
• Minimum Experience: One year of professional experience required, minimum one year experience working with the frail and elderly population, home care or geriatric experience.
• Required License: Valid Colorado Driver’s License and proof of Auto Insurance in accordance with state statute.

Physical Requirements

• Ability to lift/push/pull minimum of 35 lbs.
