Vierra Falls Church Health & Rehabilitation is hiring CNAs to join our care team in Falls Church, VA. We offer competitive pay and a supportive work environment focused on resident well-being. This role provides direct patient care with opportunities for growth.

Responsibilities include assisting residents with daily activities, meal services, weight and skin condition monitoring, and collaboration with nursing staff to maintain care plans. A CNA license is required.

Qualifications

• CNA license current and unrestricted.
• CPR/BLS certification current.
• 2 years CNA experience in long-term care is a plus.
• Proficiency with technology-driven patient care and timekeeping software.

Responsibilities

• Provides supportive services to nursing staff and performs assigned duties.
• Assists residents with activities of daily living per care plans and policies.
• Assists with transfers, mobility, positioning, and transportation of residents.
• Coordinates dining room services at meals, delivers trays, and documents intake.
• Delivers nutritional supplements and tracks intake as required.
• Monitors resident weight and reports changes to nurse/supervisor.
• Assists with skin condition monitoring and toileting program activities.
• Completes flow sheets and communicates progress to supervisors.

Major Duties and Responsibilities (include but are not limited to):

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists resident with or performs activities of daily living for resident in accordance with care plans and established policies and procedures.
• Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
• Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.
• Assists with weighing residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor.
• Assists with tracking the condition of the resident's skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor.
• Assists nursing staff in carrying out toileting program activities.
• Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor.
• Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e. resident refusal, reassigned to nursing unit).
• Reports changes in a resident's condition to the resident's nurse and supervisor.

Required Qualifications:

• CNA License – current & unrestricted
• High school diploma or equivalent
• CPR/BLS Certification – current
• 2 years prior experience as a CNA in long-term care a plus!
• Requires proficiency in technology-driven patient care and HRIS and timekeeping/attendance software
