Cadence Care Network is seeking a Care Coordination Supervisor (CCS) to lead recruitment and development of care coordinators. The CCS will ensure that all program goals are achieved and will be responsible for providing training and performance evaluations for the team.
The ideal candidate should have a Master’s degree, licensed in accordance with relevant codes, and possess at least three years of supervisory experience in children's behavioral health or related sectors.

The Care Coordination Supervisor (CCS) will participate in the recruitment, interviewing, and training of potential new care coordinators. The CCS will ensure achievement of monthly and yearly goals for staff and program, in addition to completion of GKR and monthly radar reports.
Provide both formal and informal training for assigned staff. Conduct performance evaluations and develop growth plans for assigned staff for continued professional development. Provide weekly supervision.
Assure all required ODM documentation and program requirements are completed by Care Coordinators and review program specific paperwork to ensure ongoing quality with documentation and care planning.
