Job summary

Cadence Care Network is seeking a Care Coordination Supervisor (CCS) to lead recruitment and development of care coordinators. The CCS will ensure that all program goals are achieved and will be responsible for providing training and performance evaluations for the team.

The ideal candidate should have a Master’s degree, licensed in accordance with relevant codes, and possess at least three years of supervisory experience in children's behavioral health or related sectors.

Qualifications

• Minimum of three years' supervisory experience in children's behavioral health or related fields.
• Licensed individual according to rule 51602701 of the Administrative Code.
• Experience in family systems, community systems, and resources.

The Care Coordination Supervisor (CCS) will participate in the recruitment, interviewing, and training of potential new care coordinators. The CCS will ensure achievement of monthly and yearly goals for staff and program, in addition to completion of GKR and monthly radar reports.

Provide both formal and informal training for assigned staff. Conduct performance evaluations and develop growth plans for assigned staff for continued professional development. Provide weekly supervision.

Assure all required ODM documentation and program requirements are completed by Care Coordinators and review program specific paperwork to ensure ongoing quality with documentation and care planning.

Qualifications

• Master's degree
• Licensed individual in accordance with rule 51602701 of the Administrative Code.
• A minimum of three years' supervisory experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related sector of human services or behavioral health care field, providing communitybased services to children and youth, and their family or caregivers.
• Experience in on or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection or child development.
• Pass preemployment drug screening and ODJFS required criminal records check.
