Qualifications

• Minimum of one year working in a medical office setting.
• Certification in Medical Assisting (AAMA or CAAHEP) is required; CHW certification preferred; CPI and BLS certifications must be obtained after hire.

Responsibilities

• Prepare accurate and comprehensive patient charts for medical appointments.
• Coordinate care and track care–related data, including medication assistance and referrals.
• Complete prior authorizations for medications and referrals as needed.
• Track referrals and follow up to ensure timely, documented care.
• Schedule referral appointments and support continuity of care across services.
• Document all patient interactions in the electronic health record in real time.
• Maintain HIPAA-compliant documentation and professional boundaries.
• Support UDS measures and social determinants of health screening as assigned.
