Job summary

Qualifications

• High School diploma or GED is required.
• Minimum of one year working in a medical office setting.
• Certification in Medical Assisting (AAMA or CAAHEP) required or preferred; CHW certification preferred; CPI and BLS obtained after hire.

Responsibilities

• Prepare accurate and comprehensive patient charts, identifying care gaps and relevant prior test results.
• Track and coordinate patient care–related data, including medication assistance and samples, and other care documentation.
• Complete prior authorizations for medications and referrals as needed.
• Track and manage closed-loop referrals, ensuring timely follow-up and accurate documentation.
• Schedule referral appointments while patients are on-site to support continuity of care.
• Coordinate return appointments across medical, behavioral health, and dental services.
• Document all patient interactions accurately and in real time within the electronic health record.
• Maintain HIPAA-compliant documentation and professional boundaries.
