You will be responsible for coordinating pre-release and post-release case management for CCC inmates with mental health, critical care medical, and substance use disorder needs to ensure a smooth transition to the community and services. Provide in community support for at least 30 days after release to ensure continuity of care and reduce recidivism. Create a discharge plan with goals that reflect the needs of the inmate.
You will collaborate and coordinate with Chesapeake Integrated Behavioral Health’s community-based services and community partners to refer and link inmates with resources to support community re-entry, to include transitional housing or other housing support, and minimize recidivism.
The discharge plan examples are assisting inmates with a SMI resident with obtaining photo identification, birth certificates, apply for Medicaid and/or GAP applications/reinstatement, apply for Social Security disability/ SSI assistance, assist with emergency food and clothing as approved through the Inmate Jail Program Fund, etc.
