Job summary

City of Chesapeake Virginia seeks an Early Intervention Case Manager to support infants and toddlers (birth to three) and their families through the Infant & Toddler Connection of Chesapeake.

The clinician will coordinate intake, eligibility, service planning, and ongoing monitoring, ensuring families access needed resources and services to promote development and participation in daily activities.

Qualifications

  • Requires a bachelor’s degree in social work, counseling, psychology, or a closely related field.
  • Two years of related, full-time experience required.
  • May require QMHP/QIDP certification, CPR/First Aid, and other certifications within six months of hire.

Responsibilities

  • Provide early intervention services to infants and toddlers birth to three and support families.
  • Serve as service coordinator from intake through transition, including eligibility and service plan development.
  • Monitor services with monthly contacts and coordinate resources and supports for families.

Education

Bachelor’s degree in social work, counseling, psychology or related field
Master’s degree in Social Work, Counseling, or Rehabilitation (required for some programs)

Job Description

The Infant & Toddler Connection of Chesapeake is an early intervention program that builds upon and provides supports and resources to assist family members and caregivers to enhance their children’s learning and development through everyday learning opportunities.The Clinician II will be providing early intervention services to enable young children to be active and successful participants during the early childhood years; and in the future by enabling families to provide care for their child and have the resources they need to participate in their own desired family and community activities.

The Clinician II position is a service coordination/case management position specializing in working with developmentally delayed infants and toddlers, birth to three years of age, and their families, in accordance with Part C of the Individuals with Disabilities Education Act.

Clinician II/ Service coordinators work with their clients and families from the point of referral assignment through transition. Service coordinators facilitate, coordinate, and execute the following activities: intake visit, assessment for eligibility and service planning, eligibility determination, service plan development and review, monitoring services with monthly contacts, and transition planning for future needs and other resources.

Required Qualifications

Vocational/Educational Requirement: Requires a bachelor’s degree in social work, counseling, psychology, or a closely related field or other degree as approved by the Virginia Department of Health Professions. A master’s degree in Social Work, Counseling, or Rehabilitation is required for some assigned programs.

Experience: In addition to satisfying the vocational/educational standard, this class requires a minimum of two years of related, full-time equivalent experience.

Special Certifications and Licenses: Requires a valid driver’s license and a driving record that is in compliance with City Driving Standards. Depending on assignment, may require Qualified Mental Health Professional (QMHP) or Qualified Intellectual Disability Professional (QIDP) certification and highest Part C standards within 6 months of hire; CPR, First Aid and medication administration certifications within 3 months of hire, Certified Substance Abuse Counselor (CSAC) certification upon hire, Case Management Modules and Intellectual Disabilities Workbook completed within thirty days of hire, Part C Early Intervention Certification within six months of hire, and/or Pre‑Screening Certification within 6 weeks of hire (12 weeks for part-time staff).

Special Requirements: Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties

Preferred Qualifications

Early Intervention certification and experience preferred.

A master’s degree in Social Work, Counseling, or Rehabilitation is preferred.
