Job summary

Berkshirejobs is seeking a licensed clinician to provide clinical recovery services in the DMH funded Adult Community Clinical Services program. You will lead clinical teams to develop assessments and treatment plans to promote self-sufficiency.

The role involves short-term counseling, meeting with clients within 72 hours of referral, and developing Crisis and Risk Management and Rapid Housing plans. Weekend availability may be required; supervision and trainings are part of the duties.

Qualifications

• Master's Degree in Social Work, Psychology, Mental Health Counseling or other clinical concentration.
• Massachusetts LCSW, LICSW, LMHC, O.T. or active RN license.
• Knowledge of the Recovery Model.
• Knowledge of community resources.
• Proficient computer skills and data entry into spreadsheets and word processing documents.

Responsibilities

General Description

Provide clinical recovery services to Persons in the DMH funded Adult Community Clinical Services program as part of the Integrated Care Team. Also classified as a Licensed-Practitioner of the Healing Arts (when applicable), this Clinician will lead clinical teams to develop comprehensive assessments and treatment plans that encourage Persons to gain skills to transition through level of services towards self-sufficiency.

Essential Job Functions

• Provide short term individual and group counseling services
• Attend all transitional and clinical treatment meetings in the community (enrollment, intake, discharge, transfer, DMH, BH CP, etc.). Develop and monitor transition plans to support Persons experiencing life changes.
• Provide clinical oversight to Persons on the Integrated and GLE teams
• Meet the documentation of program standards (ACA, Treatment Plans, Quarterly/Annual Reviews, Assessments, referrals), completing all necessary paperwork as required through the ACCS and BH CP programs.
• Develop Person-Centered Treatment Plans that incorporate attainable and achievable goals, which are individually strength based, including personal preferences, incorporating the cultural and linguistic needs of the Person and include person LAR’s and support network (Family, Friends, Clergy, etc.)
• Meet with referred individuals within 72 hours of referral to develop immediate needs and to evaluate current risk
• Develop Crisis and Risk Management Plans
• Develop Rapid Housing plans to incorporate a Housing First Model
• Coordinate the completion of required evaluation tools (Self Sufficiency Matirx, Milestones of Recovery-MOR’s, SBRIT, risk assessments, etc.)
• Develop and facilitate clinical based trainings to Integrated Teams and GLE staff
• Provide Medication Administration to Persons per MAP protocol. This may include medication administration to cover staff emergencies and coverage needs. Audit medications as assigned.

Other Responsibilities

• Participate in regularly scheduled supervision
• Attend required meetings and agency trainings

Qualifications

• Master's Degree in Social Work, Psychology, Mental Health Counseling or other clinical concentration or an Associates or higher in Nursing
• Massachusetts LCSW, LICSW, LMHC, O.T. or active Registered Nursing license
• Knowledge of the Recovery Model
• Knowledge of community resources
• Proficient computer skills in using web based e-mail and ability to data enter information into spreadsheets and word processing document
