Job summary

Fairview Health Services is seeking a Clinical Treatment Coordinator to partner with physicians and the interdisciplinary team to guide patients and families through treatment episodes, including assessment, planning, discharge, and community re-entry. The role emphasizes coordinating care, developing individualized plans, and ensuring regulatory compliant documentation.

The CTC will work closely with hospital staff to optimize patient flow, manage length of stay, and support transitions to home

Qualifications

  • Master's degree in psychology, counseling, marriage and family therapy, or related field.
  • Doctorate in psychology, counseling, marriage and family therapy, or related field.
  • Master's or Doctoral degree in Social Work (MSW/DSW).
  • Completion of Behavioral Control Situation training (and retraining every two years or as often).
  • 2 years therapy, case management, or assessment experience with adults or children/adolescents.
  • Basic Life Support certification upon hire.
  • Licenses: LICSW, LP, LPCC, or LMFT upon hire.

Job description

Job Overview

The Clinical Treatment Coordinator (CTC) will partner with the physician and interdisciplinary team to navigate the patient/family through treatment episode, including assessment, treatment planning, discharge planning and community re-entry. This includes, but is not limited to: managing patient/family expectations, responding to the crisis of hospitalization, supporting the discharge and community re-entry needs and monitoring and managing patient length of stay and avoidable days.

Responsibilities
  • Demonstrates clinical competence in the treatment planning process.
  • Coordinates treatment planning and leads management of each case.
  • Coordinates/facilitates the development of treatment plan by partnering with the physician and collaborates with patient, family and interdisciplinary team.
  • Develops an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Implements, reassesses and adjust treatment plan as necessary.
  • Demonstrates clinical competence to assess individual patient needs.
  • Completes an interdisciplinary assessment tool with each patient.
  • Updates interdisciplinary assessments tool each week.
  • Demonstrates clinical competency to partner with inter-disciplinary team in the treatment planning process.
  • Assists in developing an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Implements, reassesses and adjusts treatment plan as necessary.
  • Communicates frequently and effectively with interdisciplinary team to ensure continuity and coordination of care.
  • Documentation is thorough, timely, legible and clinically appropriate and meets regulatory requirements.
  • Demonstrates competence in developing a clinically relevant transition/discharge plan.
  • Partners with physician and family to provide leadership to the inter-disciplinary team
  • Collaborates with patient, family, and provides input to interdisciplinary team in developing a discharge plan based on age, social and cultural considerations, clinical diagnosis and level of functioning and other relevant needs.
  • Demonstrates the ability to advance evidence based practice by utilizing standards, guidelines and pathways for care delivery. Incorporates data and information into practice to continually improve care and practice so that patient outcomes are enhanced.
  • Promotes a culture of evidence based practice by participation in any of, but not limited to, the following activities:
  • Utilizing evidence based policies in clinical practice
  • Attends and participates in learning opportunities about evidence based practice
  • Membership in hospital or unit based practice council/research council
  • Evidence based practice project (e.g. policy development or poster presentation)
  • Journal club, journal article critique and circulation
  • Literature review
  • Development of standard/pathway/guideline for patient care
  • Presentation
  • Research study
Required Qualifications
  • One of the following degrees
    • Master of Arts in psychology, counseling, marriage and family therapyor a related field.
    • orDoctorate in psychology, counseling, marriage and family therapy or a related field.
    • orMaster of Arts in Social Work orDoctorate of Social Work
  • Completion of Behavioral Control Situation training, and retraining every two years or as often
  • 2 years therapy, case management, or assessment experience, including providing face to face therapeutic/clinical services to adult, or child/adolescent clients
  • Basic Life Support (American Heart Assoc, Red Cross or MTN) Upon Hire
  • One of the following Licenses
    • Licensed Independent Clinical Social Worker (LICSW) Upon Hire or
    • Licensed Psychologist (LP) Upon Hire or
    • Licensed Professional Clinical Counselor (LPCC) Upon Hire or
    • Licensed Marriage and Family Therapist (LMFT) Upon Hire

The posted pay range reflects a 40-hour workweek (1.0 FTE) and aligns with the minimum and maximum pay rates specified in the union agreement at the time of job posting. The pay rate for an individual within this range may vary based on factors such as FTE, skills, knowledge, relevant education, and qualifying experience as detailed in the contract.
