Job summary

Fairview Health Services is seeking a licensed clinician to conduct diagnostic assessments under supervision, facilitate therapy for patients and families, and lead treatment planning with an interdisciplinary team. The role involves coordinating discharge planning and communicating with community providers to ensure seamless care.

The position requires a Master’s degree in psychology or social work, plus graduate internship completion and MN Board licensure as LGSW or progression toward

  • Completion of clinical education and internship as part of graduate program.
  • Be licensed or provisionally licensed by MN Board of Social Work as LGSW, or pursuing independent LMFT/LPCC licensure.
  • Ability to develop individualized treatment plans with interdisciplinary teams.
  • Experience with documentation and regulatory compliance.

Job description

May conduct diagnostic assessment under the supervised practice of fully licensed and system credentialed supervisor, determines clinical interventions, facilitates therapy and provides education for patients and their families. Facilitates treatment planning and manages each case. Responsible for development of transition/discharge plan. Communicates effectively and respectfully with others, including interdisciplinary teams and all other contacts. Functions as an interdisciplinary team member. Accepts supervision and consultation with a licensed supervisor for the functions of this position in their clinical practice.

Job Expectations

Demonstrates clinical competence in the treatment planning process.

  • Facilitates treatment planning and oversees management of each case.
  • Coordinates/facilitates the development of a treatment plan by collaborating with patient, family and interdisciplinary team.
  • Develops an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Implements, reassesses and adjusts the treatment plan as necessary.

Demonstrates clinical competence to assess individual patient needs.

  • Completes an interdisciplinary assessment tool with each patient.
  • Updates the interdisciplinary assessment tool each week.

Demonstrates clinical competence in disposition planning.

  • Interfaces with government agencies such as court, ombudsmen, county services.
  • Completes the referral process for the next level of care.
  • Communicates to other providers in the community.
  • Sets up intake appointments.
  • Completes needed paperwork to facilitate this process (e.g., applications, letters of recommendation, etc.).

Demonstrates clinical competency to partner with multi‑disciplinary team in the treatment planning process.

  • Assists in developing an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Implements, reassesses and adjusts the treatment plan as necessary.
  • Communicates frequently and effectively with the interdisciplinary team to ensure continuity and coordination of care.
  • Documentation is thorough, timely, legible and clinically appropriate and meets regulatory requirements.

Demonstrates competence in developing a clinically relevant transition/discharge plan.

  • Collaborates with patient, family, and provides input to interdisciplinary team in developing a discharge plan.
  • Develops a discharge plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Documents thoroughly, timely, legible and clinically appropriate.

Demonstrates clinical competence in conducting group therapy, family therapy and psycho‑education groups.

  • Provides individual, couples and family therapy.
  • Documents thoroughly, timely, legible and clinically appropriate.

Demonstrates the ability to advance evidence‑based practice by utilizing standards, guidelines and pathways for care delivery.

  • Utilizes evidence‑based policies in clinical practice.
  • Attends and participates in learning opportunities about evidence‑based practice.
  • Serves as a member of a hospital or unit‑based practice council or research council.
  • Runs or participates in evidence‑based practice projects (e.g., policy development, poster presentations).
  • Engages in journal club, critiques, and literature reviews.
  • Develops standards, pathways, or guidelines for patient care.
  • Leads presentations or shares findings.
  • Conducts or contributes to research studies.

  • Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served, partnering with patient caregivers in decision making, communicating respectfully, ensuring a safe environment, individualizing plans, modifying interventions, and providing education based on assessment of learning needs.
  • Fulfills all organizational requirements including completion of necessary learning, compliance with laws, regulations, and policies.
  • Participates with supervision by attending individual and/or group supervision as agreed upon.
  • Performs other duties as assigned.
Required

Minimum Qualifications to Fulfill Job Responsibilities:

Education: Master’s Degree in Psychology, Social Work, or a closely related field.

Experience: Completion of clinical education and internship as required by a graduate program.

License/Certification/Registration: Be licensed or provisionally licensed by the MN Board of Social Work as LGSW, or be working toward independent licensure as an LMFT or LPCC.
