Fairview Health Services in Minneapolis, MN seeks a licensed clinician to determine interventions, facilitate therapy, educate patients and families, and manage diagnostic assessments.
You will develop treatment plans, coordinate transitions/discharges, and collaborate with an interdisciplinary team to support patient care.
Applicants hold a master's in Social Work or Psychology and licensure (LICSW, Psychologist, LPCC, LMFT) with at least two years of experience.
Determines clinical interventions, facilitates therapy, and provides education for patients and their families. Completes Diagnostic assessment and bills under a physician. Facilitates treatment planning and manages each case. Responsible for development of transition/discharge plan. Communicates effectively and respectfully with others, including interdisciplinary team and all other contacts. Functions as an interdisciplinary team member.

The posted pay range reflects a 40-hour workweek (1.0 FTE) and aligns with the minimum and maximum pay rates specified in the union agreement at the time of job posting. The pay rate for an individual within this range may vary based on factors such as FTE, skills, knowledge, relevant education, andqualifying experience as detailed in the contract. If your role is eligible for a sign-on bonus, the bonus program that is approved and in place at the time of offer, is what will be honored.
