Job summary

Option Care Health is seeking a Clinical Transition Specialist to educate patients and hospital staff about infusion services and facilitate patient placement at home or alternate sites. You will build relationships with referral sources, collaborate with discharge planners and account teams to grow referrals and meet sales goals.

This role requires RN or Pharmacist licensure with 2+ years in healthcare, strong communication, and the ability to coordinate across internal and external partners to

Qualifications

• Licensed RN or Pharmacist in the practicing state with at least 2 years in healthcare.
• Proven relationship-building and customer service skills across diverse stakeholders.
• Familiar with ACHC/URAC standards and related regulatory requirements.
• Strong PC skills including Word, Excel and PowerPoint.
• Ability to identify issues and implement process improvements.
• Reliable transportation with valid driver's license and insurance.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Serves as key point of contact and representative to provide education, assistance, and service.
• Interacts with area service providers to sell Option Care services for discharged patients.
• Educates patients, families and facility staff about services and products.
• Coordinate with Account Manager and Regional Sales Director on growth plans.
• Ensure timely discharge and therapy independence through effective planning.
