Job summary

Hackensack Meridian Health is seeking a Clinical Therapist I to join a multidisciplinary team, providing psychosocial assessments and interventions to support patients and families across behavioral/medical health contexts.

The role requires a Master’s degree in Social Work or Counseling, 0–3 years of related experience, and licensure supervision. Responsibilities include treatment planning, counseling, documentation, and collaboration for discharge planning.

Qualifications

• Master's degree in Social Work (MSW) or Counseling or related field
• 0-3 years of relevant work experience in Social Work or related fields
• Supervision under a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) with approved supervision or ACS credential
• Ability to perform psychosocial assessments and develop treatment plans
• Knowledge of crisis intervention theory and its applications
• Knowledge of disease management and differential diagnosis and treatment techniques
• Team player with strong communication skills
• Proficient computer skills (MS Office/Google Suite)

Responsibilities

• Provides psychosocial assessments and develops treatment plans under supervision
• Delivers behavioral/medical health treatment and education (individual and/or group)
• Completes all documentation per policy
• Intervenes per licensure regulations and provides information to staff/families
• Assesses and interprets information for patient needs and treatment options
• Provides case management as warranted
• Assesses Social Determinants of Health (SDOH) and offers referrals
• Collaborates with patient and family to develop discharge plans
• Reassesses treatment plans and updates as needed
• Maintains patient health information per regulations
• Assists team during emergencies
• Identifies community resources for patients
• Adheres to organizational competencies and supervision requirements
• Additional duties as assigned
