The role involves assessment, counseling (individual/group), care planning, and discharge planning across settings, under licensed supervision. Requires MSW or related field, LCSW/LPC supervision, and CPI certification.

Qualifikationen

• Master's degree in Social Work or related field.
• 0-3 years of social work experience.
• Supervision by a licensed clinician with approved supervision.
• Ability to perform psychosocial assessments and develop treatment plans.
• Knowledge of crisis intervention theory and applications.
• Familiarity with disease management and differential diagnoses.

Aufgaben

• Conduct psychosocial assessments and develop treatment plans under supervision.
• Provide behavioral/medical health treatment and education to patients.
• Complete documentation per policy.
• Deliver services per licensure regulations.
• Collaborate with care teams to develop discharge plans.
• Assess Social Determinants of Health and refer as needed.
