Job summary

Benchmark Transitions, part of Emend Healthcare, is seeking clinical therapists to provide therapy, manage a caseload of up to 12–14 clients, and support treatment teams. The role includes family therapy, crisis intervention, and comprehensive documentation in the EMR.

The therapist will collaborate with the Clinical Director and leadership to deliver individualized care, coordinate with referral sources, and participate in ongoing staff training and program development.

Qualifications

• Duties include leading or participating in groups and managing a caseload of up to 12–14 clients.
• Provide family therapy and guidance to clients and families.
• Maintain written documentation in EMR and communicate with treatment teams.

Responsibilities

• Proactively develop client treatment plan and monitor progress.
• Provide routine communications to treatment team and referral sources.
• Provide enrollment orientation and discharge planning as appropriate.
• Maintain accurate EMR documentation within 24 hours.
• Support learning independent living skills and other care components.
• Provide crisis intervention as needed and determine transfer needs.
• Participate in on-call rotations and floor duties when required.
• Assist new staff to learn Benchmark processes.
• Ensure clinical services are delivered flexibly for better engagement.
• Adhere to Benchmark standards and incident reporting policies.
