Benchmark Transitions, part of Emend Healthcare, is seeking a Clinical Therapist to deliver therapy, manage a caseload, and support families in our Montclair, CA program.

You will collaborate with the Clinical Director and leadership, conduct crisis intervention, and complete timely EMR documentation to support ongoing care and program development for clients and their communities. This role requires flexibility to cover weekend shifts and emergencies as needed.

Qualifications

  • Licensed clinical therapist with active credentials.
  • Experience providing individual and family therapy.
  • Familiarity with Joint Commission standards.

Responsibilities

POSITION PURPOSE

Clinical therapists provide therapy and therapeutic processes to Benchmark Transitions clients and oversee/assist all therapy-related services. The therapist is a member of the clinical department which provides overall clinical support for our clients and their families and referral agents. This includes leading or participating in various groups, effectively managing a caseload of up to 12 to 14 clients for individual therapy, providing family therapy and guidance, and providing written or phone updates to proper outside and referral sources on a regular and systematic basis. The role of the therapist is to also assist in helping our clients to embrace other parts of their treatment program, which includes learning independent living skills as well as academic or vocational pursuits. The therapist further provides crisis intervention, clinical assessments and monitoring for self-harm, group safety levels, and emotional readiness for events/activities. The therapist is responsible for all written documentation which must be stored at the appropriate time in our EMR system. The therapist is to work collaboratively to establish positive interaction with co-workers, clients and Benchmark customers. The therapist receives clinical direction from the Clinical Director and works in collaboration with the Chief Operations Officer and Program Director and other executive leadership to form a cohesive approach for our clients and families.

Requirements

This is a 24/7/365 operation. We are prioritizing applicants who can work Saturdays and/or Sundays. In addition, this position may be required to work mandatory overtime and/or extended shifts for emergencies and operations.

DUTIES AND RESPONSIBILITIES
  • Proactively develop client treatment plan and monitor for client involvement and progress. Provide interventions, and facilitation of group and individual and family sessions.
  • Provide routine communications to the treatment team, appropriate family members, referral sources, utilization review specialists, and other related professionals as needed.
  • Provide enrollment orientation and discharge planning as appropriate within time limits.
  • Must provide routine and accurate documentation in EMR files pertaining to each client and the continuum of care process and do so within the time allotted, (24 hours).
  • Provide guidance to clients and participate in program development for Benchmark clients. An example would be to support learning independent living skills or other care components such as learning animal care and horsemanship skills, work skills and such tasks that are outside of the normal treatment modalities.
  • Provide crisis intervention as needed and determine transfer needs if applicable.
  • Must participate in our emergency/administrator on-call rotations and assist with floor duties if possible.
  • Provide assistance to new staff to help them learn our processes at Benchmark.
  • Clinical therapists must demonstrate flexibility in delivery of services to adjust where adjusting would provide better therapeutic alliance.
  • Clinical therapists must display ability to exercise sound judgment and be able to display the use of independent discretion in dealing with young adult issues and agree to bring all concerns to their supervisors and executive staff.
  • Clinical therapists must display strong professional yet interactive interpersonal skills and work in a safe manner as well as recognize that Benchmark can often be a fast-paced environment.
  • Provide support, troubleshooting of problems and administration of solutions in accordance with clinical concerns to our clients.
  • Clinical therapists must adhere to all Benchmark standards.
  • Clinical therapists must maintain mental and emotional stability yet be able to express assertive personality when necessary.
  • Clinical therapists must know State Licensing and Joint Commission rules and regulations and be willing to participate in reviews prompted by either.
  • Clinical therapists must keep their certification active and current.
  • Therapists are team players and should help train new staff to orient them to our processes.
  • Occasionally therapists may be required to accompany clients to outside events. Examples might be a field trip, or court or visit the client’s independent living quarters.
  • Therapists will encourage clients to agree to the Benchmark agreements and encourage the development of positive client leadership.
  • Clinical therapists work collaboratively with other staff to ensure a safe and supervised milieu including support in the classroom and elsewhere.
  • Therapists agree to support and comply with all incident/injury policies and appropriately report any unsafe practices or situations or concerns to the proper department or leadership team.
  • Clinical therapists to ensure the mission and philosophy of Benchmark Transitions is clearly communicated, represented, demonstrated and reflected within all program operations and interactions.
  • Clinical therapists may occasionally be asked to attend conferences or present lectures to related professional development as requested therefore some travel should be expected.
  • Assist with occasional duties as needed to ensure the collection of client urine.
  • Clinical therapists may be asked to assist in other non-clinical duties as needed, such as instruction of any non-clinical classes, (teach creative arts, or other classes that benefit our clients or fill in for any absent non-clinical position).
  • Help research ways to continually improve our system of delivery of services.
  • Be on call on a rotational basis to support essential business clients and emergency situations.
  • Provide purchase justifications by turning in receipts.
  • Perform other duties as assigned within the scope of appropriate duties.
