This position is responsible for addressing the psychosocial outpatient needs of patients and their families by providing clinical social work evaluation, individual, family, and group counseling, and cultural diversity and advocacy expertise linked to community resources. This position develops, implements, coordinates, and evaluates client service plans in an outpatient environment; collaborates with other health care providers, community service agencies, and faith-based services as appropriate to coordinate care and services. This position requires full understanding and active participation in fulfilling the Mission of Blessing Hospital. It is expected that the employee demonstrate behavior consistent with the Core Values while supporting the strategic plan, goals, and direction of the Performance Improvement Process (PIP) at Blessing Hospital.
