Job summary

Midwest Health is seeking a Clinical Support RN to serve as the primary clinical responder and escalation resource. You will provide real-time guidance, clinical decision-making, and care coordination to support communities and residents as needed.

In collaboration with Executive Directors and RCCs, you will respond to calls, document interactions, and advise on appropriate levels of care while communicating with physicians, families, and staff. Proficiency with EMR systems is expected.

Qualifications

• Must hold a compact/multistate LPN license.
• Experience providing real-time clinical guidance or escalation support.

This person will work with the Executive Directors and RCCs (Resident Care Coordinator) to do the following:

• Perform real-time guidance as part of quality assurance and respond to incoming calls and alerts

• Manage and document complex clinical situations (falls, change in condition, behaviors)

• Advise on appropriate level of care and recommend for resident interventions

• Communicate with physicians, families, and staff

• Document interactions in EMR or other software

• Review, assess, educate, and provide guidance on senital events and participate in the QAPI program

This person have a compact/multistate Licensed Practical Nurse (LPN) license.
