Job summary

Delta-T Group seeks a Clinical Supervisor Supports Coordinator in Dearborn, MI. This contract role focuses on person-centered planning for an assigned caseload, including goal provision, guardianship and Medicaid coordination, and linking with supports to maximize independence.

Responsibilities include monthly supports coordination contacts, clinical notes, and maintaining compliant case records. A minimum of three years in mental health/disability services and LMSW or LPC are required; MS

Qualifications

• Minimum of three years of professional experience in mental health, developmental, or disability services.
• Proficiency in Microsoft Office required.
• LMSW or LPC licensure is required.
• Valid Michigan Driver License is required.

Responsibilities

• Provision and monitoring of person centered planning to an assigned caseload, including identifying and assessing dreams/desires/needs during the plan and goal provision process.
• Assist with managing guardianships and Medicaid/entitlements.
• Coordinate with Skills Trainers and Employment Representatives on Vocational Profile/Employment Plan.
• Develop individualized person centered plan of service and link with supports; monitor and amend as needed.
