Job summary

21st Century Home Health Services is seeking a Clinical Supervisor - Nursing to oversee RN Case Managers and LVNs in a hybrid field-and-office role in the Northern/Santa Clara Bay Area. You will report to the Director of Nursing and ensure coordinated, high-quality home care across a defined coverage area.

The role requires a CA RN license, BLS, and at least two years of supervisory experience, with strong leadership abilities and commitment to clinical excellence.

Qualifications

• Two years' management or supervisory experience within health care, preferably Home Health.
• Current CA Registered Nurse License (RN).
• Current Basic Life Support (BLS) Certification.
• Valid CA Driver's License with clean driving record. Licenses/certifications must be current at hire and maintained.

Responsibilities

• Oversee field clinicians who provide care to patients in their homes.
• Serve as first point of escalation; ensure timely support for patient care and staff.
• Coordinate training and educational programs for nurses in the field to improve outcomes.
• Plan and monitor bi-weekly Case Conferences with the clinical team; revise care plans as needed.
• Review cases by analyzing services provided, conferences, and records; guide clinicians to improve performance.
