Full-Time Clinical Supervisor: Nursing

Coverage area: Oakland to Solano/Napa

Reporting to the Director of Nursing, you will coordinate patient care with RN Case Managers and LVNs, serve as a first point of escalation, develop nursing educational programs, and maintain client relationships by providing updates and insights to improve patient outcomes.

This role involves a hybrid of fieldwork and office work at the Walnut Creek Office, with 2 days/week in the office and 3 days/week in the field.

Key Responsibilities:

1. Oversee clinicians providing home care.
2. Handle patient activities and follow-ups.
3. Coordinate training for nurses.
4. Assess staff needs and provide coaching.
5. Assist clinicians with patient care escalations.
6. Conduct bi-weekly case conferences.
7. Review and guide case management.
8. Establish therapeutic goals with clinicians.
