Multiple Full-Time Clinical Supervisor: Nursing Opportunities!

Territory: San Francisco County to Redwood City

In this leadership role, reporting to the Director of Nursing, you will coordinate patient care managed by RN Case Managers and LVNs. You will serve as the first escalation point, support clinical staff, and participate in developing and evaluating nursing educational programs to promote clinical excellence and staff development. Regularly, you will meet with clients to maintain relationships and provide updates on patient outcomes and readmission reports.

This role involves a hybrid of field work and on-site responsibilities at the 21HHS office:

Burlingame Office: Spend 2 days per week in the office for administrative and team management duties.

Fieldwork: Spend approximately 3 days per week (average of 12+ hours) conducting supervisory visits and providing direct support to RN Case Managers.

Responsibilities of the Clinical Supervisor - Nursing

• Oversee nurses providing care at patients' homes.
• Handle patient calls promptly and follow up appropriately.
• Coordinate training and educational programs for field nurses.
• Assess staff learning needs, set goals, and serve as a resource and role model.
• Assist clinicians with patient care escalations.
• Conduct bi-weekly case conferences to review patient progress and update care plans.
• Review clinical cases, verify documentation, and guide clinicians to improve performance and care quality.
• Review patients' diagnoses, medications, procedures, and clinical course.
• Assist clinicians in establishing therapeutic goals and developing care plans.

Qualifications for Success

• Minimum two years' management or supervisory experience in healthcare, preferably in home health, within the last five years
• Graduate of an accredited nursing program; Bachelor’s Degree preferred
• Current CA RN license
• Current BLS certification
• Valid CA driver’s license with a good driving record; all licenses and certifications must be current and maintained throughout employment

$143,000 - $163,000 annually, plus a quarterly bonus of $40,000. Starting salary depends on experience. We offer bonus incentives, comprehensive health benefits, and a 401k plan with up to 4% match.
