Job summary

Baptist Health in Kentucky is seeking a Clinical Staff Pharmacist for full-time 3rd shift. You will ensure safe, effective medication therapy, support diverse medication use systems, and provide pharmacologic expertise to care teams across units.

Responsibilities include antimicrobial stewardship, patient counseling, and collaboration with physicians and nurses to optimize transitions of care and discharge planning.

Qualifications

• Graduate of an accredited college or university with Pharmacy degree.
• Licensed Pharmacist with the Kentucky State Board of Pharmacy in good standing.
• Pharm.D. or relevant clinical hospital pharmacy experience is desirable.
• Must provide documentation of state licensure and CE as required by the state of KY.
• Maintain BLS & ACLS certifications
• Advanced clinical training or PGY1 residency preferred

Responsibilities

• Provide safe, effective, and prompt medication therapy.
• Support centralized and decentralized medication use systems.
• Deliver optimal medication therapy to patients with a broad range of disease states.
• Perform drug preparation, input, verification, and inventory management.
• Serve as drug information resource and consultant to providers and staff.
• Engage in antimicrobial and opioid stewardship and patient counseling.
• Collaborate with inter-professional teams on transitions of care and discharge planning.
• Participate in unit-based huddles and monitor unit metrics.
