Job summary

SILVERCREEK is seeking a Speech-Language Pathologist to evaluate and treat residents, coordinate with families and physicians, and document services for medical records. The role includes training staff, supervising CFY or students, and ensuring safe, compliant practice in a healthcare setting.

The staff SLP will develop treatment plans, monitor progress, and collaborate with an interdisciplinary team to optimize residents’ communication and swallowing abilities and discharge planning.

Qualifications

• Licensed as a Speech-Language Pathologist in the state.
• Must maintain continuing education per state requirements.
• Bachelor's degree in Speech-Language Pathology; advanced degree preferred.

Responsibilities

• Evaluate residents to determine speech therapy needs and plan care.
• Develop and update individualized treatment plans for communication and swallowing.
• Provide training for facility staff and supervise CFY/students as needed.
• Communicate progress and discharge goals with families and health teams.
• Complete accurate documentation per facility and payer requirements.
• Maintain safe treatment environment and infection control standards.
