• Collaborate with healthcare team to provide services and optimize patient functioning.
• Conduct comprehensive evaluation of social, emotional, cultural, mental status, environments and financial circumstances with interdisciplinary assessment.
• Recommend a plan of intervention based on mutually established patient goals.
