Job summary

Division of Child and Family Services is hiring a full-time Clinical Social Worker I for the Intensive In-Home Clinical Services Team. This role provides short-term, high-intensity, home- and community-based interventions for youth with complex behavioral health needs.

The position supports youth returning home from treatment and those needing intensive stabilization. Responsibilities include leading individual and family therapy, crisis stabilization, safety planning, and coordinating care

Qualifications

• Master's degree in social work from an accredited college or university.
• Internship approved by the Board of Examiners for Social Workers.

Responsibilities

• Lead individual and family therapy.
• Deliver crisis stabilization and safety planning.
• Coordinate care and provide psychosocial rehabilitation and skills training.
• Perform case management and discharge/aftercare planning.
• Develop and monitor treatment plans and progress notes.
• Advocate for clients with external agencies and resources.
