UCSF is hiring a Clinical Social Worker I/II to deliver community-based case management and psychotherapy to formerly homeless clients at permanent supportive housing sites in San Francisco. You will perform outreach, crisis intervention, 5150 holds, and link clients to housing supports, medical care, and community resources.

Documentation and collaboration with housing staff and medical teams are essential.

Qualifications

• Master's degree in social work or related field.
• Knowledge of DSM-5-TR, Harm Reduction, Trauma-Informed Care, and anti-racism principles.
• Experience in community-based clinical case management, outreach, and crisis intervention.
• Ability to work with high-risk, mentally ill, and formerly homeless populations.
• Excellent communication, documentation, and collaboration skills.

Responsabilités

• Provide ongoing mental health services, psychotherapy, and crisis intervention for a caseload of formerly homeless clients in PSH sites.
• Conduct intake, assessments, treatment planning, and 5150 evaluations; coordinate with housing providers and community partners.
• Document interventions in EMR (Epic) and maintain clinical records per Medi-Cal and City requirements.
• Engage clients to increase housing retention and link to supports; collaborate with hospital and medical providers.
• Attend clinical trainings, supervision, and team meetings; uphold cultural humility and anti-racism in practice.
