Job Description

The Clinical Social Worker I/II (CSW) provides comprehensive psychosocial assessment, crisis intervention and 5150 services, counseling, brief therapeutic interventions, consultation, education, outreach, community linkages, and facilitates the plan of care for patients with particular focus on high-risk populations. Additionally the incumbent provides mental status examinations and diagnoses using the Diagnostic Statistical Manual (DSM5-TR); consultation to a multi-disciplinary treatment team; acts as liaison to hospital and community resources; maintains client records according to California State Medi-Cal and San Francisco City and County requirements; record data as required by Department of Public Health, attends staff meetings and participates in training sessions and performs other duties as assigned. Under the general supervision of the Clinical Supervisor, the Clinical Social Worker I/II works within the innovative Citywide Stabilization Program, which utilizes a proactive, client-centered approach to engage and treat adults with severe mental illness who are often dually diagnosed and unhoused. This program will be responsive to the needs of the Mental Health San Francisco (MHSF) initiative and will be fluid in service delivery based on the needs of the Behavioral Health Services under the Department of Public Health. The CSW I/II works collaboratively with the behavioral health system of care to develop creative treatment strategies. The incumbents job duties may include but not limited to below:

Intensive Case Management: The incumbent carries a caseload of high risk and vulnerable patients that are struggling to engage with necessary mental health care. The service delivery will include low threshold intensive case management, stabilization, and linkage to the appropriate level of care.

CARE Court: This position works with clients involved in the Citywide CARE Court program, referred by the Department of Public Health CARE Court Program, and provides intensive case management services to clients for the duration of their time in CARE Court.

Assisted Outpatient Treatment (AOT)- Provides intensive case management to adults that are either court ordered to treatment or voluntarily agreed to treatment after meeting the legal threshold for AOT. These individuals require specialized treatment engagement.

DEPARTMENT DESCRIPTION:

This position is a part of the UCSF Department of Psychiatry and Behavioral Sciences Community Services Citywide Case Management Division, on the Citywide Stabilization Team.

Qualifications

• MINIMUM REQUIREMENTS:
• CSW I: Master's degree in social work, psychology or related field from an accredited university and registered with the Board of Behavioral Science as An Associate Clinical Social Worker (ASW) or Intern Marriage Family Therapist (IMFT); and knowledges and abilities essential to the successful performance of the duties assigned to the position.
• CSWII: Master's degree in social work, psychology or related field from an accredited university and possession of the requirements necessary for application for licensure as determined by the California Board of Behavioral Science; and knowledges and abilities essential to the successful performance of the duties assigned to the position.
• Ability to work cooperatively and communicate effectively with others. Ability to work in a high stress, high volume area and work well under pressure and with deadlines. Ability to follow oral and written instructions accurately, and be detailed oriented. Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes. Knowledge of medical and/or psychiatric terminology.
• Excellent communication and interpersonal skills.
• Able to work as a team member in a culturally diverse setting.
• Able to lift 50lbs and climb stairs.
• Demonstrated excellent attendance and reliability.
• A preliminary job offer and assignment may be extended pending successful background clearance.
• An understanding of mental health issues and sensitivity toward mental health clients.
• Ability to work with clients and program staff from diverse cultural and social class backgrounds.
• Two years working with severely mentally ill adults, the homeless and dual diagnosed populations
• One year experience doing community based case management and the ability to do outreach including to SRO hotels
• Case Management experience PREFERRED:
• Prior related UC experience
• Capacity to work with clients in a second language
• LICENSES/CERTIFICATIONS:
• CSW I: Master's degree in social work, psychology or related field from an accredited university and knowledge and abilities essential to the successful performance of the duties assigned to the position.
• CSW II. A Master's degree in social work, psychology or counseling from an accredited school of social work and possession of the requirements necessary for application for licensure as a clinical social worker, as determined by the California Board of Behavioral Science Examiners; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position in addition to all the above apply.
