Odyssey House in New York, NY seeks a Social Worker to perform clinical assessments, individual and group counseling, and participate in multidisciplinary care. Qualifications

• Master’s Degree in Social Work, Counseling, or Marriage and Family Therapy required.
• One year of experience in social services with group counseling, family therapy, and case management preferred; NYS licensure preferred.
• Proficiency with Microsoft Word, Excel, EHRs, and Outlook.

Responsibilities

• Complete clinical assessment and re-assessments to measure progress.
• Provide individual counseling addressing family, medical, psychiatric, and housing concerns.
• Formulate and monitor treatment plans with clients; update plans per regulatory requirements.
• Conduct evidence-based group counseling across clinical and community topics.
• Maintain up-to-date resident charts and documentation of contacts.
• Provide crisis intervention services.
• Assist in discharge planning.
• Coordinate with utilization review to ensure insurance authorization documentation.
• Establish community and agency affiliations to address client needs.
• Educate families and provide direct services as appropriate.
• Maintain caseload when needed and conduct screenings for contraband/Toxicology testing.
• Attend staff meetings, case conferences, and training; participate in QA and quality improvement.
