Intermountain Health is seeking a Social Work Care Manager I in Riverton, UT. The role performs psychosocial assessments, develops care plans with the care team, and provides crisis counseling and referrals. Collaboration with patients, families, providers, insurers, and community resources is essential.

This PRN position includes weekend and holiday shifts. A $500 sign-on bonus is offered for external applicants with 1+ year of experience.

Qualifications

• Master of Social Work from an accredited institution.
• Current state licensure (CSW/LCSW) where applicable.
• Proficiency with standard software tools.
• Strong written and verbal communication skills.
• Ability to apply critical thinking in care planning.
• Caregivers with home/community visits must meet driver/insurance requirements.

Responsabilités

• Assesses patients psychosocial needs and screens for transition planning.
• Develops and monitors care plans addressing social determinants of health.
• Provides brief individual, family, and group therapy when indicated.
• Identifies mental, emotional and behavioral disorders and connects to services.
• Educates and advocates for patients to facilitate safe transitions of care.
