Qualifications include an MSW, CSW/LCSW licensure, and strong communication. Preference for experience in care management and health-insurance processes; BLS and reliable transportation required where applicable.

Qualifications

• Master of Social Work from an accredited institution (degree verification required).
• Current state licensure (CSW or LCSW) prior to or upon supervision hours completion.
• Basic computer proficiency with word processing and spreadsheets.
• Strong written and verbal communication skills.
• Ability to apply critical thinking.
• Home/community visit requirements include valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Conduct psychosocial assessments and develop care plans with the care team.
• Provide individual, group, and family therapy as indicated.
• Coordinate transitions of care and connect to services.
• Assess crisis situations and provide counseling and referrals.
• Educate and advocate for patients and families.
• Collaborate with insurers and community providers to ensure quality care.
• Lead improvement initiatives and ensure policy adherence.
• Facilitate goals of care discussions with patients and families.
