Job summary

Intermountain Health is seeking a Social Work Care Manager I (PRN) at McKay-Dee Hospital in Ogden, Utah. You will perform psychosocial assessments, develop care plans with the care team, provide crisis counseling, and connect patients and families to community resources.

Collaborating with healthcare providers, insurers and other partners, you will support transitions of care, advocate for patient access, and lead caregiver education while maintaining compassionate care.

Qualifications

• MSW from an accredited institution (degree verification required).
• Current state licensure (CSW/LCSW) prior to/supon supervision hours.
• Basic computer proficiency for word processing and spreadsheets.
• Strong written and verbal communication skills.
• Ability to apply critical thinking.
• Caregivers with home/community visits must maintain BLS and have a valid license, insurance and reliable transportation.

Responsibilities

• Assessment & Screening: evaluates patients for transition planning, mental health, substance use, and goals of care.
• Care Coordination: develops and monitors care plans addressing social determinants and resources.
• Behavioral Health Support: uses motivational interviewing and therapy for mental health including women's services and trauma.
• Therapeutic Intervention: provides brief individual, group, and family therapy and psychosocial assessments.
• Diagnosis & Referrals: identifies disorders and connects to services.
• Education & Advocacy: trains staff, educates patients, and facilitates safe transitions of care.
• Team Collaboration: works with providers, insurers, and community providers for quality care.
• Quality & Compliance: leads improvement initiatives and ensures policy adherence.
• Advanced Care Planning: facilitates goals of care discussions with patients, families, and teams.
