Job summary

Integritus Healthcare seeks a licensed social worker to join the care team in Massachusetts. The role focuses on psychosocial assessments, family/caregiver counseling, crisis intervention, and coordinating care through discharge planning.

The position requires MA licensure (LCSW/LICSW) and at least one year of healthcare experience; experience in long-term care is preferred. Strong communication and organizational skills are essential.

Qualifications

• Minimum 1 year of experience in health care.
• Experience in a long term care setting preferred.
• Massachusetts licensure: LCSW or LICSW.

Responsibilities

• Assess the psychosocial status of patients and communicate findings to the care team.
• Conduct psychosocial evaluations addressing family dynamics, caregiver abilities, stress, and coping; plan interventions and provide crisis support.
• Educate interdisciplinary team on cultural needs of patients and families, including family roles and traditions.
• Maintain clinical records for patients referred to social work; ensure Point of Service documentation and timely data synchronization.
• Provide information and referrals to patients and families/caregivers and community agencies.
• Serve as liaison between patients, families/caregivers and community agencies.
• Participate in discharge planning and development of the care plan; attend interdisciplinary group meetings.
