Job summary

IntegriNurse is seeking a licensed social worker (LCSW/LICSW) in Massachusetts. The role focuses on psychosocial assessment, family counseling, crisis intervention, and collaboration with the interdisciplinary care team in a long-term care setting.

The ideal candidate has at least one year of healthcare experience, strong communication, and organizational skills, with a preference for candidates with long-term care exposure and a Bachelor’s degree.

Qualifications

• Minimum of one year experience in health care.
• Experience in a long-term care setting preferred.
• Current Massachusetts licensure: LCSW or LICSW.
• Demonstrates good verbal and written communication, and organization skills.

Responsibilities

• Assess psychosocial status and communicate findings to the care team.
• Counsel patient and family/caregivers and provide crisis intervention as needed.
• Educate on cultural needs and family dynamics, traditions or taboos.
• Maintain clinical records and ensure timely Point of Service documentation.
• Provide information and referrals for practical and environmental needs.
• Serve as liaison between patients, families and community agencies.
• Participate in care planning and discharge planning as needed.
