Job summary

Qualifications

Responsibilities

• Provides biopsychosocial assessments and clinical diagnoses.
• Interviews patient and social support to complete evaluation.
• Provides brief psychotherapy and education to patient and family.
• Develops and revises a care plan with the multidisciplinary team.
• Monitors and documents patient/family education and progress.
• Advocates for patient and family throughout care continuum.
• Provides crisis intervention when needed and participates in clinical pathways.
