This position is bonus eligible, with earning potential up to 15% of your annual salary!

Responsibilities
  • Receive case referrals, review patient information, and assign clinicians accordingly.
  • Evaluate cases, review clinician services, and guide staff to ensure quality home care services.
  • Review patient diagnoses, medications, procedures, and clinical progress.
  • Assist clinicians in establishing therapeutic goals and developing care plans.
  • Conduct quarterly record reviews and communicate findings to leadership.
  • Contribute to policy development and strategic planning.
Qualifications
  • Licensed clinician with current licensure in the state.
  • Graduate of an accredited school; BSN/DPT/MOT/MSLP preferred.
  • Minimum of two years management or supervisory experience in healthcare, preferably home care.
  • Current CPR certification.
  • Valid driver’s license and proof of insurance.
