TOUCH OF HEALING HANDS HOME CARE LLC in Lumberton, NC is seeking a Qualified Professional (QP) to oversee clinical services, assess clients, develop PCPs, and ensure Medicaid-compliant, person-centered plans.

The role involves coordinating with families and providers, supervising staff, maintaining documentation, and traveling to client homes as needed; NC Qualified Professional qualifications and licensure apply.

• Monitor client progress and evaluate the effectiveness of services.
• Coordinate services with families, guardians, providers, and community resources.
• Maintain confidentiality of client records and ensure Medicaid guidelines are followed.
