Job summary

Qualifications

• Master’s degree in social work, psychology, counseling, or a related Behavioral Health field.
• Eligible for Georgia licensure as LMSW/LCSW, APC/LPC, LAMFT/LMFT; must pursue licensure upon hire.

Responsibilities

• Conduct comprehensive biopsychosocial and behavioral health assessments.
• Develop, implement, and monitor individualized treatment and discharge plans.
• Coordinate client transitions and discharge planning to ensure continuity of care.
• Maintain accurate clinical documentation and releases of information.
• Communicate with community partners, courts, parole/probation, and treatment teams.
