River Edge Behavioral Health in Macon, GA seeks an experienced Clinical Service Coordinator to join our Adult Outpatient team. The role conducts assessments, plans care, coordinates services, and supports discharge planning, with option for telehealth and in-person visits.

The position requires a Master’s in a behavioral health field and Georgia licensure eligibility, with active pursuit of licensure after hire. Full-time with competitive benefits and supportive culture.

Qualifications

• Master’s degree in social work or related Behavioral Health field required.
• Eligible for Georgia licensure as LMSW/LCSW, APC/LPC, LAMFT/LMFT.
• Must actively pursue professional licensure upon hire.

Responsibilities

• Conduct biopsychosocial and behavioral health assessments.
• Develop, implement, and monitor individualized treatment and discharge plans.
• Assess client appropriateness for outpatient services and referrals.
• Collaborate with treatment teams to develop interventions and strategies.
• Coordinate transitions and maintain clinical documentation.
• Communicate with community partners and stakeholders as needed.
• Prepare reports and participate in monthly meetings.
• Maintain productivity with billable hours.
